DELUSIONS

Ask the following questions:

1. Is there a history of drug or alcohol ingestion? Many drugs may be associated with delusions, especially cocaine, PCP, and LSD. The confabulations of Korsakoff's psychosis may be confused with delusions.
2. Is there impairment of memory? If there is impairment of memory, an organic psychosis should be suspected, such as senile and pre-senile dementia or general paresis. When there is no impairment of memory, the problem is probably a psychiatric disorder such as schizophrenia or manic-depressive psychosis.

DIAGNOSTIC WORKUP

If there is a history of alcohol or drug ingestion, a blood alcohol level and urine for drug screen should be done. A CT scan, EEG, and spinal fluid analysis will often need to be done before referring a patient to a psychiatrist for further evaluation.
