Diagnostic tests for Shock:

Various tests are used in the diagnosis of Shock. Some of these are listed below :

  • Physical examination
    • Hydration status - dehydration signifies major fluid loss such as hemorrhage, vomiting , severe diarrhea, burns, heat exhaustion or pancreatitis.
    • Temperature - Temperature may be elevated initially with septic shock, but may be reduced in later stages of septic shock. Lowered temperature is particularly common in very young or elderly people with septic shock. Temperature may also be elevated with cardiogenic shock from myocarditis or heart attack.
    • Blood pressure. Reduced blood pressure is a late sign in septic and hypovolemic shock. In the young and fit, the blood pressure may remain normal. There will be reduced blood pressure with cardiogenic shock
    • Heart rate and irregularity, noting any arrhythmias. Elevated heart rate is a reliable sign of shock in the young. It is not so reliable in the elderly or in people taking beta blocker or calcium blocker blood pressure medication

Diagnosis Checklist: Questions your doctor may ask (and why!)

During a consultation, your doctor will use various techniques in their assessment of Shock. Your doctor may ask several questions when assessing your condition. It is important to remember that your consultation is a two-way process and any extra information you can share with your doctor about your symptoms may help them with their diagnosis. Some of the questions your doctor may ask are listed below:

  • What do you mean by shock? - Shock is difficult to define. The term "shock" is used to describe the physical and mental reaction to reduced blood circulation resulting in inadequate blood supply to body tissues.
  • What is the cause of the shock? - There are 4 major causes of shock including hypovolemic shock (e.g. blood loss, fluid loss), septic shock (e.g. bacterial, fungi or viral infection), anaphylactic shock (e.g. allergic reaction) and cardiogenic shock (e.g. heart attack, heart arrhythmia, pulmonary embolism, pericardial tamponade, cardiomyopathy).
  • Fluid intake? - may suggest inadequate intake of fluid and a contributor to hypovolemic shock e.g. poor fluid intake due to illness, anorexia, malnutrition or neglect.
  • History of trauma? - may suggest hypovolemic shock due to bleeding.
  • Past history of high blood pressure? - Blood pressure is reduced in shock relative to the person's previous blood pressure e.g. an elderly person whose usual blood pressure is 185/95 may be in shock with a blood pressure of 130/85. It is thus important to establish what the person's blood pressure was prior to becoming unwell.

  • Vomiting? - may suggest the source of hypovolemic shock e.g. hyperemesis gravidarum, viral gastroenteritis, food poisoning, meningitis, cyclic vomiting syndrome, pyloric stenosis (projectile vomiting), intestinal obstruction, cholera, peritonitis and pancreatitis.
  • Diarrhea? - may suggest the source of the hypovolemic shock e.g. viral gastroenteritis, cholera, food poisoning, ulcerative colitis, short bowel syndrome, amoebic dysentery.

Shock: (1) The condition in which the cells of the body receive inadequate amounts of oxygen secondary to changes in perfusion; most commonly secondary to blood loss or sepsis. (2) A sudden physical or biochemical disturbance that results in inadequate blood flow and oxygenation of an animal's vital organs. (3) A state of profound mental and physical depression consequent to severe physical injury or to emotional disturbance.
