Misdiagnosis and Posture symptoms

Scapulocostal syndrome: The Scapulocostal syndrome is a common musculoskeletal, pain-producing syndrome that mainly affects the posterior shoulder area, but, because of radiation, characteristics it can mimic numerous other conditions including cervical radicular pain, intrinsic shoulder joint disease and even visceral pain. It can be diagnosed clinically with a careful history and physical examination. There are no related blood test abnormalities or imaging abnormalities in the syndrome, but these tests may be useful in eliminating other entities from diagnostic consideration. Diagnostic tests for Posture symptoms:

Various tests are used in the diagnosis of Posture symptoms. Some of these are listed below :

  • Physical examination
    • Examine for any spinal deformity such as scoliosis (spine is curved to the side and sometimes twisted) or thoracic kyphosis (excessive front curvature of the upper thoracic spine)
    • Examine for signs of Parkinson's disease such as lack of facial expression, bent over posture, few spontaneous movements, shuffling gait, lack of normal arm swinging with walking, pill-rolling tremor (where movement of the knuckles are combined with movements of the thumb), increase tone of limbs, monotonous speech
  • Blood tests
    • Full blood count and ESR
    • Electrolytes and kidney function tests
    • Arthritis screen depending on suspicion including Rheumatoid factor, ANA and ENA antibodies

Adults are more likely to suffer from osteoarthritis, Paget's disease, Parkinson's disease, osteomalacia, osteoporosis and ankylosing spondylitis.
  • Risk factors for osteoporosis? - e.g. female sex, thin build, family history, early menopause, smoking, chronic alcoholism, high caffeine intake, low calcium intake, intestinal malabsorption, physical inactivity, prolonged corticosteroid use, Cushing's syndrome, diabetes mellitus, hyperthyroidism, hyperparathyroidism, Acromegaly, chronic renal failure.
  • Past cancer history? - of cancers that may spread to bones e.g. breast, lung , prostate, thyroid, kidney, bladder, adrenal, melanoma and colorectal.
  • Past medical history? - e.g. osteoporosis, Paget's disease of the bone, kyphosis, scoliosis, Parkinson's disease, Rickets . Scoliosis may be due to unknown causes or due to poliomyelitis, cerebral palsy, muscular dystrophy or neurofibromatosis. Kyphosis may be associated with osteoporosis of the spine, ankylosing spondylitis and Scheuermann's disease.

  • Symptoms of Parkinson's disease? - e.g. stooping posture, coarse hand tremor most marked at rest, rigidity of limbs, slowness in initiating and executing movements and speech, expressionless mask-like face and dementia (in later stages).
