Poor growth: Introduction

Normal growth is important for infants, children and adolescents. A slowing of growth rates can indicate various medical problems, even if it does not lead to weight loss. In other words, it is not enough to have some growth, but the growth rate must be enough for the normal rate. Growth rates and metrics such as weight, height, and head circumference should be evaluated against average ranges, and any change in the percentile ranking assessed carefully. Any growth abnormality needs early professional medical diagnosis, especially for infants and children.

Misdiagnosis and Poor growth

Coarctation of aorta in Turner's syndrome: Turner syndrome is a sex chromosomal abnormality due to the presence of only one X chromosome occurring in newborn females. The genetic profile is 45 chromosomes of XO karyotype. It is characterized by retarded growth that leads to a small stature and frequent infertility. Turner syndrome is diagnosed on the basis of genetic analysis of chromosomes. This can be done prior to birth. Most individuals with Turner syndrome require female hormone therapy to promote development of secondary sexual characteristics and menstruation Most women with Turner syndrome can live relatively normal lives. The prognosis for a person with Turner syndrome is dependent on other conditions that may be present. Early surgical intervention is required to treat webbed neck. It is also important to differentiate Turners syndrome from Noonan's syndrome which has a few similar features. In Noonan's syndrome mental retardation is usually present and the cardiac lesion associated is pulmonary stenosis which is not the case in Turner's syndrome as mental retardation occurs very rarely and the main cardiac pathology involved is coarctation of aorta....read more »

    • Looking for any dysmorphic ( abnormality of shape) features of face and body that may affect feeding or be associated with poor growth - e.g. Down syndrome, Pierre Robin syndrome (small jaw, cleft palate and downward displacement of the tongue), fetal alcohol syndrome (narrow space between the eyes, short nose, small face, small upper lip), Zellweger syndrome.
    • Observe the child's behavior and interaction with yourself. If child is alert, interested in new objects, keen to explore and relates in a friendly manner, a growth problem is more likely because of an isolated condition rather than global retardation
