Diagnostic tests for Hemiparesis:

Various tests are used in the diagnosis of Hemiparesis. Some of these are listed below :

  • Physical examination
    • Blood pressure - if blood pressure is high must consider a stroke due to a cerebral hemorrhage, cerebral thrombosis or cerebral aneurysm.
    • Pulse rate and rhythm looking for atrial fibrillation which would suggest a cerebral embolism as a cause of a stroke
    • Cardiovascular examination looking especially for murmur and enlarged heart and carotid artery bruit
    • Full neurological examination to help determine the site or level of the lesion in the brain or spinal cord that caused the hemiparesis. Neurological examination must include cranial nerve examination, examination of tone, power, reflexes, coordination and sensation of both the upper and lower limbs looking for signs of a stroke, brain tumor or multiple sclerosis
  • Blood tests
    • Full blood count and ESR

Diagnosis Checklist: Questions your doctor may ask (and why!)

During a consultation, your doctor will use various techniques in their assessment of Hemiparesis. Your doctor may ask several questions when assessing your condition. It is important to remember that your consultation is a two-way process and any extra information you can share with your doctor about your symptoms may help them with their diagnosis. Some of the questions your doctor may ask are listed below:

  • How long have you had hemiparesis (one sided paralysis of the body)? - to determine if acute or chronic.
  • Is the hemiparesis intermittent? - would suggest migraine, transient ischemic attacks from the basilar artery or carotid artery disease, epilepsy.
  • Was the onset sudden or gradual? Sudden onset would most commonly suggest a stroke due to cerebral thrombosis, hemorrhage or embolism; however a contusion or concussion of the spinal cord may also produce a sudden onset of hemiparesis. If there is a history of trauma and a sudden onset of hemiparesis must consider a subdural or epidural haematoma. A gradual onset of hemiparesis would suggest a brain tumor or brain abscess
  • History of trauma? - would suggest a subdural or epidural haematoma or a hemorrhage in the brain itself.
  • Recent viral infection? - e.g. measles, chicken pox, mumps, rubella - may suggest Acute disseminated encephalitis.

Sometimes, other symptoms may be present and may help your doctor analyse your condition. The symptoms your doctor may ask about (in association with Hemiparesis) may include:

  • Headaches? - may suggest brain cancer or brain metastasis, subdural or extradural haematoma, stroke.
  • Symptoms of transient ischemic attack? - e.g. sudden onset of neurological deficit that lasts for less than 24 hours (average length of time is 5 minutes) with complete clinical recovery. Consciousness is usually preserved. Symptoms may include sudden blindness in one eye; inability to speak; weakness, paralysis or numbness of one side of the face, arm and leg.
