Blindness: Introduction
Any type of vision loss (e.g. blindness, blurred vision, double vision, etc.)
is a symptom of great concern.
Many of the causes are very serious medical conditions.
Certain types of vision changes can be a medical emergency
where delay can lead to loss of sight (e.g. for causes such as glaucoma, eye injury, retinal detachment)
or loss of life (e.g. for causes such as stroke, TIA, etc.).
Even transient or temporary blindness or loss of vision cannot be ignored
because it can
result from serious conditions such as stroke, TIA, hypertension, epilepsy, or
migraine.
Seek immediate professional medical attention for any such symptoms of vision changes.

Glaucoma-red eye: Acute angle closure glaucoma is a medical emergency and can very often be misdiagnosed when it makes an atypical presentation.It occurs due to sudden increase in the intra ocular pressure. Patients may complain of sudden onset of severe pain in the affected eye with redness,nausea,vomiting ,headache and reduced vision.All cases of red eye must visit the opthalmologist the same day.Sometimes the cases may have more systemic presentations than ocular and hence the diagnosis maybe missed which can lead to blindness....read more »

Diagnostic tests for Blindness:
Various tests are used in the diagnosis of Blindness. Some of these are listed below :
- Physical examination
- Blood pressure - high blood pressure increases risk of hypertensive retinopathy, stroke and central retinal vein thrombosis.
- Visual acuity - reflects the integrity of the central field.
- Visual field testing - to detect either a contraction (a local or general reduction in the area of field) or a scotoma (an island of loss within the field). Use Confrontation testing, Perimetry testing and Central field testing. Mapping the visual field defect can help determine cause.
- Examination of pupils
- Ophthalmoscopy and slit lamp examination - to examine the optic nerve, macula and retina and assist in diagnosis of corneal opacities, cataracts, choroiditis, glaucoma, retinitis, retinal hemorrhage, detached retina. Specifically look for papilledema (swelling of the optic nerve) which suggests optic neuritis, retinal vein thrombosis and brain tumors.

During a consultation, your doctor will use various techniques in their assessment of Blindness.
Your doctor may ask several questions when assessing
your condition. It is important to remember that your consultation is a two-way process and any extra information
you can share with your doctor about your symptoms may help them with their diagnosis.
Some of the questions your doctor may ask are listed below:
- Is blindness transient? - may suggest transient ischaemic attacks, epilepsy, migraine and high blood pressure.
- Is the blindness unilateral or bilateral? - unilateral blindness may occur in glaucoma, vitreous hemorrhage, optic neuritis, retinal vein thrombosis, central retinal artery thrombosis, carotid artery thrombosis, temporal arteritis, injury to the optic nerve, fractured skull, brain tumor, retinoblastoma and sphenoid ridge meningioma. Bilateral blindness may occur in posterior cerebral artery occlusion, pituitary tumor, retinitis pigmentosa, hereditary optic atrophy, uveitis, toxic amblyopia, cataracts, glaucoma, multiple sclerosis and iritis.
- Was the onset of blindness sudden or gradual and progressive? - Sudden onset of blindness may occur in optic neuritis, retinal vein thrombosis, central retinal artery occlusion, vitreous hemorrhage, detached retina, carotid artery thrombosis, temporal arteritis, injuries to the optic nerve, retrobulbar neuritis, fracture of the skull, glaucoma, posterior cerebral artery occlusion, multiple sclerosis and hysteria.
- If blind spot and not total field of sight blindness, where in the line of sight do you notice the blind spot? - e.g. central blind spot suggests macular disease or optic neuritis; glaucoma and retinitis pigmentosa can cause peripheral visual loss.
- Past medical history? - e.g. diabetes increases risk of cataracts and retinal detachment; multiple sclerosis and syphilis can cause optic neuritis; migraine can cause transient blindness.
