Infection from the spirochete Treponema pallidum causes syphilis. Transmission occurs primarily through sexual contact during the primary, secondary, and early latent stages of infection. Prenatal transmission from an infected mother to her fetus is also possible. (See Prenatal syphilis.)
Incidence is highest in people ages 20 to 29.

- Congenital syphilis is transmitted from an infected mother to her unborn or newborn baby.
- Acquired syphilis is sexually transmitted from an infected to an uninfected individual.
- Infection transmitted to the fetus at any stage of disease; during primary and secondary syphilis, rate of transmission is 60–100%
- Nasal secretions are highly infectious in congenital syphilis, and open, moist skin lesions are infectious in congenital and acquired syphilis.

The term 'prevalence' of Syphilis usually refers to the estimated population
of people who are managing Syphilis at any given time.
The term 'incidence' of Syphilis refers to the annual diagnosis rate,
or the number of new cases of Syphilis diagnosed each year.
Hence, these two statistics types can differ:
a short-lived disease like flu can have high annual incidence but low prevalence,
but a life-long disease like diabetes has a low annual incidence but high prevalence.
