Nephrotic syndrome: Signs and symptoms
(Professional Guide to Diseases (Eighth Edition))

The dominant clinical feature of nephrotic syndrome is mild to severe dependent edema of the ankles or sacrum, or periorbital edema, especially in children. Edema may lead to ascites, pleural effusion, and swollen external genitalia. Accompanying symptoms may include orthostatic hypotension, lethargy, anorexia, depression, and pallor. Major complications are malnutrition, infection, coagulation disorders, thromboembolic vascular occlusion, and accelerated atherosclerosis.

Nephrotic syndrome: Signs and symptoms
(Handbook of Diseases)

  • Reduced appetite
  • Weight gain and facial swelling
  • “Puffy eyes”
  • Abdominal swelling or pain
  • Urine that is foamy
  • Atopy
  • Pitting-dependent edema
  • Fluid accumulation in body spaces (ascites, pleural effusions, scrotal swelling)
  • White nails, lusterless hair, soft ear cartilage
  • Hepatomegaly
  • Mild hypertension

The signs and symptoms of glomerular disease include

  • Proteinuria: large amounts of protein in the urine.
  • Hematuria: blood in the urine.
  • Reduced glomerular filtration rate: inefficient filtering of wastes from the blood.
  • Hypoproteinemia: low blood protein.
  • Edema: swelling in parts of the body.
One or more of these symptoms can be the first sign of kidney disease. But how would you know, for example, whether you have proteinuria? Before seeing a doctor, you may not. But some of these symptoms have signs, or visible manifestations:
  • Proteinuria may cause foamy urine.
  • Blood may cause the urine to be pink or cola-colored.
  • Edema may be obvious in hands and ankles, especially at the end of the day, or around the eyes when awakening in the morning, for example.
